Waiver | Phenomenally360

In consideration of being allowed to ride BeYouPhenomenally360 the undersigned on his or her behalf and the behalf of the undersigned participants agrees to the follwing conditions. Please check the following to acknowledge...:: Required

I agree that the participant(s) and I shall comply with all stated and customary terms, posted safety signs, rules and verbal instructions as conditions for participation with BeYou-Phenomenally360. In addition, if I observe any hazard during our participation I will bring it to the attention of the nearest BeYoutifully Blessed LLC employee or official immediately.

I am aware that there are inherent risks associated with participation in BeYoutifully Blessed LLC equipment the BeYou-Phenomenally 360. I, on behalf of myself and the participant(s), knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence of other participants.

Myself and the partcipant(s) and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless BeYoutifully Blessed LLC, their affiliates, employees, other partcipants from and against any and all claims, injuries, liabilities, death, or damages arise out of or related to our participation in BeYou-Phenomenally360.

I acknowledge to BeYoutifully Blessed LLC that I, the participant(s) do not have any physical limitations, medical ailments, or physical or mental disabilities that would limit or prevent the participant(s) from participating in the activity. If required, the participant(s) will obtain a medical examaination and clearance.

I agree with BeYoutifully Blessed LLC that, I and participant(S) is forever prevented from suing or otherwise claiming against BeYoutifully Blessed LLC for any property loss or personal injury that the participant(s) may sustain while participating in or preparing for BeYou-Phenomenally360.

I agree with BeYoutifully Blessed LLC that, I was given the opportunity and have been encourage to seek independent legal advice prior to signing this waiver.

I agree that BeYoutifully Blessed LLC can bill my card on file $3,000 for loss of items or damage due to improper or abusive use of the items.

I agree with BeYoutifully Blessed LLC that, BeYoutifuly Blessed LLC has given me sufficient time to carefully read this waiver.

I agree with BeYoutifully Blessed LLC that, I fully understand the risks and claims that i am waving to participate in BeYou-Phenomenally360, and I am freely and voluntarily executing this waiver.
